How to create a form

Build forms that you can attach to projects.

In molti you can create custom forms to capture data. On the form page, you can drag-and-drop different field types to create an interactive form that you can then add to projects.

Some of our fields are:

  • Text Box

  • Check Box

  • Select Drop-Down

  • Nested Select Drop-Down

  • Radio Button List

  • Check Box List 

  • Formulas

... and more!

To build a simple form:

  1. Go to the Forms page in the left-side Navigation Menu.

  2. If you want to create a simple form, select the blue + Add Form  option in the top right corner. 

  3. Name and create your form.

  4. Build your form by dragging and dropping your fields.  Remember you can only drop fields to the blue highlighted area.  

  5. Once you're done building select Done  to save.  Now you can attach your form to projects to collect data!

If you want to take advantage of some more advanced fields, such as "Linked Fields" you need to use a "Form Group".  Linked Fields are specialty fields that will "link" or display information from another field in another form.  However, in order for these to work, both forms need to belong to the same "Form Group".  The groups are organizational and let the system know the forms are connected.

To build a form and form group:

  1. Go to the Forms page in the left-side Navigation Menu.

  2. Select the blue + Add Group option in the top right corner.

  3. Name and create your form group.

  4. Select the blue + Add Form  option in the top right corner.

  5. Now you're ready to build! Build your form by dragging and dropping your fields.  Remember you can only drop fields to the blue highlighted area.  

  6. Once you're done building select Done to save.  Now you can attach your form to projects to collect data!
